Team Decision Making Meetings
DYRS holds a Team Decision Making (TDM) meeting with the youth, the family, the case manager, care coordinators and other service providers. DYRS asks this group to come together to determine the best treatment and rehabilitation plan for the youth, based on his or her individual strengths and needs. Based on this input along with information about the youth’s offense and placement history, the case manager develops an Individual Success Plan (ISP) for the youth. The Individualized Success Plan describes the services the youth needs, such as tutoring, job training, or substance abuse prevention, and the progress the youth needs to make.
